Cogent Staffing are looking for a Care Coordinator to join our clients team in Halifax, handling childhood immunisations.

The successful candidate will travel independently between 4 different practices in North Halifax, working on a rota basis.

Shifts & salary:

  • 12 month fixed term contract
  • Monday to Friday
  • 30 – 37.5 hours a week
  • £23,000 salary

Key responsibilities:

  • Undertake responsibilities in line with practice demand for childhood vaccinations.
  • Provide education and advocacy about the benefits and safety of vaccinations, addressing any concerns in misconceptions individuals and their carers.
  • Work with practices to facilitate easy access to immunisation services.
  • Track immunisation rates and follow up with families.
  • Build and maintain relationships with patients and families with consideration to health inequalities and diversity.
  • Participate in outreach activities to raise childhood vaccination awareness and uptake.
  • Work closely with other health, social and educational care professionals, such as
  • local schools, childcare providers, to identify and support patients making sure that access to appropriate vaccinations is available to them and ensuring that their needs are met.
  • Report on progress, challenges and outcomes to management and stakeholders.
  • Proactively identify and work with registered patients, their families and carers to support their personalised care needs in order to achieve vaccination uptake.
  • Ensure that people have good quality information to help them make educated choices to vaccinate.
  • Signpost support to patients/family/carers on information about immunisations.
  • Maintain accurate patient records at all times.

Key requirements:

  • Flexibility and ability to carry out other duties within the role as required.
  • GCSE level educated
  • Experience with customer service
  • Preferably experience working in healthcare, or social care.
  • Excellent communication skills.
  • IT skills, including Microsoft Office.
  • Able to work under pressure successfully.
  • Able to follow procedure and policy.
